Who you gonna call? Case Managers!
Case Managers can go by many different names; Discharge Planners, Life Coaches, Navigators, Population Health Managers, Social Workers, or Care Managers. But one thing remains the same, the care and compassion that every case manager gives to their clients.
After a catastrophic accident the world can be full of questions. A case manager is the proverbial, “person behind the curtain” in these situations. They are the ones who are working behind the scenes to make sure that you have everything you need to be successful.
Some case managers work in the hospital setting, here they will help find rehabilitation options, order medical supplies, gather appropriate clinical documentation, communicate with ALL of your medical staff, and if needed, they will help you find placement while you heal. In these scenarios, you may only meet with your case manager a couple of times and never see them again, but in other healthcare settings, case managers can be with their clients for many years. If someone has a disability, or is having trouble navigating their daily life, it is very common that you will work with a case manager for the rest of your life. They become a part of your family, and they are your number one advocate. They help find jobs, therapies, social inclusivity activities, community groups, housing, and almost any other need that their client may have.
The job of a case manager is one that never stops. Even if they are not on the clock, a case manager is always networking and advocating. Building relationships, finding resources, and attending conferences are all a part of the case manager role that happens outside of typical work hours. These men and women are working around the clock, nights, and weekends in order to make sure that they are able to provide the best resources to their clients. If you or someone you know works with a case manager, be sure to give them some extra love this month and show them how appreciated they are! Without case managers, the healthcare world would be lost.
